Through Integrity Cares, our transitions program works to 'make life a little easier' for individuals facing advancing chronic or life limiting illness. It is a professionally managed, volunteer-based community service program designed to help individuals navigate the physical, emotional and social challenges often occurring with serious illness. Transition services include practical assistance, support and information (no hands-on nursing care is provided). Transition services are offered at no cost to community residents and their families.

Who Qualifies for

Transitions Services?

Residents of Albany, Bainbridge, Dublin, Eastman, or neighboring communities serviced by Integrity Hospice and are at least 18 years of age who are:

  • Diagnosed with advancing chronic or life limiting illness and are under the care of their physician.

  • Pursuing curative or palliative treatment.

  • Not yet ready to make a decision for hospice care.
